Real-World Adherence to Oral Anticoagulants After Hospital Discharge

Abstract

In a cohort of 2,310 patients, 31% discontinued oral anticoagulants within six months of discharge. Pharmacist-led counselling before discharge was associated with a 40% relative reduction in discontinuation.
